  2. Fill out the form: Open the PDF on your computer using a PDF editor (e.g., Adobe Acrobat). Enter information in all required fields.

  3. Download/Save the packet: Once completed, download and save the file to your device.

  4. Email the packet: Attach the saved PDF and send it to rivpsych1@hush.com

  5. Next steps: Please call RPMG at (951)275-8500 to confirm your scheduled appointment. We look forward to working with you!
